Job summary

Aspiranet is seeking a Case Manager/Life Youth Coach for its Fresno, CA office to provide case management and supportive housing services to up to 12 young adults in Transitional Age Youth programs. The role emphasizes trauma‑informed, client‑centered approaches and collaboration with a multidisciplinary team to empower youth through housing, employment, education, and wellness supports.

Qualifications

• Bachelor’s degree in a related field.
• 2 years of experience working with children and/or transition age youth and knowledge of the foster care system.
• Meets all state required conditions of employment (fingerprint, TB/health, CDL and clean driving record).

Responsibilities

• Engage Youth and support team to identify strengths and needs for service plan development and implementation.
• Provide weekly case contact and support to assess program compliance, regulatory standards, and progress.
• Conduct assessments and screenings to address identified needs and safety issues with timely referrals, follow‑ups, and safety planning.
