Responsibilities

• The social work assistant functions as a Targeted Case Manager (TCM) providing care management across Strong Ties Clinic.
• Supports individuals in accessing medical, social, legal, housing, educational, vocational services and other supports.
• Collaborates with an interdisciplinary team and community resources to coordinate care and maximize patient outcomes.
