Yellowstone Boys and Girls Ranch, Inc. is seeking a Targeted Case Manager in Missoula, MT to coordinate services for youth and families.

You will advocate, assess needs, refer to services, and develop comprehensive treatment plans with a collaborative team. The role requires strong communication, professional boundaries, and the ability to work across schools, homes, and community settings to ensure continuity of care and positive outcomes for youth.

Qualifications

• 21 years old or older
• Criminal / Child Abuse Clearance: Pass background checks as required by the State of Montana
• Education: Bachelor’s degree in human services related field; combination of experience and formal education may be considered in lieu of degree

Responsibilities

• Advocate, support, and guide youth and families in obtaining needed services, creating and implementing service plans, and monitoring services
• Maintain contact with other service providers to meet youth and family needs
• Research community services to identify best matches for youth and families
• Collaborate with youth, family and professionals to develop treatment/service plans and lead treatment team meetings
• Complete, update, and maintain documentation within COA, Montana ARM's, Contracts, and YBGR policies
• Submit and verify paperwork, including progress notes and intake forms, while supporting PQI and financial viability of the program

Job description

Position Summary: Yellowstone Boys and Girls Ranch Targeted Case Managers manage all aspects of a youth’s case while providing critical community resources to support youth and families. Targeted Case Managers provide wrap-around support for families while working collaboratively with current and prospective services to meet any youth’s complex needs.
Starting Salary Range: $20.38 - $22.94 per hour. Salary reflects steps 1-5 of an 11-step pay schedule. The starting salary will be determined based on the applicant’s education and relevant experience.
Daily And Monthly Responsibilities

• Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, assessing youth and family’s needs, referring to services, creating and implementing service plans, and monitoring services of the youth and family.
• Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
• Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.
• Collaborate the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings.
• Complete, update, and maintain all necessary documentation within the guidelines set by COA, Montana ARM's, Contracts, and YBGR Policy and Procedures.
• Complete, submit, and verify accuracy of paperwork, including but not limited to progress notes, intake paperwork, etc., while maintaining Performance Quality Improvement (PQI) and financial viability of the program.

Qualifications
Required Qualifications:

• 21 years old or older
• Criminal / Child Abuse Clearance: Must pass criminal record and child abuse registry background check as required by the State of Montana.
• Education: Bachelor’s degree in human services related field of study. A combination of previous professional experience and formal education from an accredited university may be considered in lieu of degree.

• Possess the ability to work with youth and families in multiple settings, such as within the school, home, and community.

• The ability to complete forms within an electronic Medical Health Record System typing at least 35 words per minute while using a keyboard and mouse. (Familiarity with Microsoft Office 365 products is a plus.)
