Job summary

Alameda Health System in Oakland, CA seeks a Systemwide Clinical Palliative Care Nurse Coordinator to lead cross-site assessment and coordination of palliative care. You will work with a multidisciplinary team across hospitals, clinics and SNFs to implement palliative measures and triage needs system-wide.

Responsibilities include rounds, education, advance care planning, and developing standards of care for palliative nursing across the system.

Qualifications

• Graduate of an accredited Nursing program with a Bachelors Degree in Nursing.
• One year of experience in palliative care, hospice or with the seriously ill population; five years of nursing experience in hospital, clinic, SNF and/or home care in related clinical area (oncology, geriatrics, ICU, neuro).
• Valid license to practice as a Registered Nurse in the State of California.
• Certified Hospice and Palliative Nurse (CHPN) by HPNA must be obtained two years from Date of Hire.
• Bi-lingual or multi-lingual capability, especially Spanish, Cantonese, Mandarin, Vietnamese, Tagalog, Russian.

Responsibilities

• Lead the identification and comprehensive assessment of seriously ill patients across the healthcare system.
• Ensure the appropriateness of palliative care consultation for patients in various units and facilities.
• Participate in the development of system-wide structures to support palliative care team assignments and manage patient volumes.
• Prepare and manage the morning census and staffing needs across all facilities.
• Attend morning rounds and provide insights relevant to all units.
• Conduct monthly rounds at each site to work with nursing staff on the ICU rounding tool.
• Oversee resources and staffing, triaging consults and addressing day-to-day needs system-wide.
• Monitor team notifications and communicate with palliative care team members and referring providers.
• Implement triage criteria to assign patients across the system.
• Assist team members in coordinating services with nursing, physicians and care management.
• Document assessments and report symptoms to palliative physicians/APCs and maintain records.
• Participate in advance care planning discussions and complete necessary documentation.
• Facilitate system-wide education and support for advance care planning.
• Ensure stakeholders are informed and aligned with patient goals.
• Identify learning needs of patient care staff and communicate to leadership.
• Educate patients and families about palliative care and advance care planning.
• Support clinical teams in implementing palliative orders, including pain management.
• Partner with nursing leadership to develop trauma-informed care curriculum.
• Create and implement standards of care for palliative nursing system-wide.

About this position

Systemwide Clinical Palliative Care Nurse Coordinator

Systemwide clinical care for patients referred for palliative care consultation and coordination of care across clinical sites, including hospitals, outpatient clinic, and skilled nursing facilities; works with the interdisciplinary palliative care consult team of physicians, advanced practice providers, medical learners, chaplains, and social workers to deliver patient and family care across the Alameda Health System care continuum. Work across all healthcare sites to identify areas where nursing staff need support in implementing palliative care measures.

• Participate in the development and enhancement of system-wide structures and functions to support palliative care team assignments and manage patient volumes efficiently.
• Prepare and manage the morning census, accounting for patient changes and daily staffing needs across all facilities.
• Attend morning rounds and contribute to patient discussions, providing insights and recommendations relevant to all units.
• Conduct monthly rounds at each healthcare site to work directly with nursing staff on utilizing the ICU rounding tool.
• Oversee the management of resources and staffing, triaging consults and addressing day-to-day clinical needs system-wide.
• Share the responsibility of monitoring team notifications (e.g., pager, phone) and communicate effectively with palliative care team members and referring providers.
• Implement triage criteria to assign patients across the system, ensuring immediate needs are met efficiently.
• Assist team members in coordinating services provided by palliative care and other healthcare team members, including nursing, physicians, and care management teams.
• Document assessments and report moderate to distressing symptoms to palliative physicians/APCs, maintaining consistency in patient records system-wide.
• Participate in advance care planning discussions with patients and families, completing necessary documentation (advance directives, POLST) and progress notes.
• Facilitate system-wide education and support for advance care planning.
• Ensure all stakeholders are informed and aligned with patient care goals.
• Identify learning needs of patient care staff and communicate these to nursing and program leadership system-wide.
• Provide education to patients and their families about palliative care, advance care planning, and available services across the healthcare system.
• Support clinical teams, especially nursing colleagues, in implementing palliative-related orders, including pain management and comfort care.
• Partner with nursing leadership at all sites to develop a curriculum for trauma-informed care.
• Create and implement standards of care for palliative care nursing across the entire healthcare system.
• Performs other duties as assigned.

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Minimum Qualifications

The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate’s experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization. Alameda Health System also offers eligible positions a generous comprehensive benefits program.
