Job summary

Pipeline Health System in Los Angeles is seeking a System Director of Case Management to unify four hospitals’ case management programs, drive policy development, and align performance improvement with community needs.

You will oversee directors and managers, set LOS targets, manage denial and discharge planning, report KPIs, and partner with Revenue Cycle, physicians, nursing leadership, and community partners to ensure coordinated, high-quality care.

Qualifications

• Minimum five years of clinical nursing experience required.
• Minimum five years of case management experience required; hospital setting preferred.
• Minimum three years in a supervisory capacity or higher required.
• Demonstrated ability to hire, train, coach, manage performance, and mentor case management leaders and staff.
• Strong command of regulatory compliance, utilization management, discharge planning, denials, LOS strategy, and interdisciplinary collaboration.

• Build and evolve a system-wide case management program, including policies, procedures, quality plans, and performance improvement practices.
• Partner with hospital executives, physicians, UM physician chairs, nursing leadership, the Revenue Cycle team, and community partners.
• Lead strategies tied to length of stay, avoidable delays, observation management, denial management, labor productivity, and financial stewardship.
• Provide executive reporting on KPIs, LOS, avoidable days, initiatives, and other case management performance indicators.
• Participate in UM Committees, payor JOMs, and serve as a clinical resource for the Hub when needed.

Pipeline Health operates four hospitals serving underserved communities across the Los Angeles area: East Los Angeles Doctors Hospital, Community Hospital of Huntington Park, Memorial Hospital of Gardena, and Coast Plaza Hospital in Norwalk.

This role sits at the point where clinical judgment, operational discipline, and human complexity meet. The System Director of Case Management will help ensure patients receive coordinated, high-quality care while advancing Pipeline Health’s mission to provide the care and service its communities deserve.

Pipeline Health’s case management function has grown meaningfully over the past five years, with strong hospital-based leaders and teams now ready for deeper system alignment.

The next chapter requires a leader who can bring consistency across four hospitals while honoring the realities of each community. Length of stay management, early discharge planning, uninsured patient placement, CMS education, and staffing stabilization are central priorities for the coming year.

Scope & Impact

This leader will oversee case management operations across all four Pipeline Health hospitals on a daily basis, supervising directors, managers, supervisors, and the broader case management teams.

• Advance Pipeline Health’s length of stay goals through early, disciplined discharge planning.
• Stabilize staffing needs across hospital departments and strengthen competencies for nurses newer to case management.
• Deepen team understanding of CMS regulations, compliance expectations, HIPAA, patient rights, and emergency preparedness responsibilities.
• Strengthen denial management in partnership with Revenue Cycle and improve financial decision‑making for complex placement needs.
• Model AIDET communication, equity-centered care, service excellence, and a “stop the line” mindset when patient safety is at risk.

Qualifications

The ideal candidate is a seasoned case management leader with hospital experience, clinical credibility, and the ability to unify teams without flattening local judgment. This person leads with urgency, clarity, respect, and compassion.

Compensation

Salary Range: $157,206.40 to $231,129.60 USD
