Responsibilities

• Implement and monitor patient care plans. Monitor, record, and communicate patient conditions as appropriate.
• Serve as the primary coordinator of all disciplines for well‑coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate discharge planning needs with members of the health‑care team.
• Demonstrate the knowledge and skills necessary to assess patients and identify physical, psychosocial, cultural, spiritual, and age‑related needs to provide individualized care.
• Delegate aspects of care to other nursing personnel consistent with their level of education and expertise to meet patients’ needs and maximize staff performance.
• Participate in daily interdisciplinary patient rounds, demonstrating knowledge of patients’ condition and providing appropriate input for desired outcomes.
• Reflect ANA Scope and Standards of Nursing Practice (2015), ANA Code of Ethics (2015), and ANA Social Policy Statement (2015).
• Develop, implement, and revise individualized plans of care.
• Participate in activities for professional growth and development outside of patient care.

Qualifications
