• Assess: perform comprehensive assessment of physical, social, and psychological status and gather information from interdisciplinary team.
• Plan: collaboratively develop an individualized care plan with patient and family involvement.
• Implement: coordinate and delegate care to ensure delivery per plan, manage time and resources.
• Evaluate: review outcomes, revise the plan as needed, ensure patient and family satisfaction.
• Professionalism: uphold ethical standards, compliance, documentation, and licensure requirements.
