Job summary

Qualifications

• English language proficiency and ability to communicate in writing and speaking.
• Current, full, active, and unrestricted registered nursing license in the United States or District of Columbia.
• Education: approved nursing program and equivalent licensure/certification as required by VA Handbook 5005.

Responsibilities

• Conduct early and ongoing assessment of surgical discharge needs.
• Collaborate with surgeons, nursing staff, PT/OT, social work, and case management to develop discharge plans.
• Educate patients and caregivers on post-surgical care, wound care, and activity restrictions.
• Coordinate referrals for home health, durable medical equipment, or outpatient rehabilitation.
• Identify barriers to discharge and coordinate with social work to address them.
