Job description

Sea Mar Community Health Centers is a Federally Qualified Health Center (FQHC) founded in 1978 that serves diverse communities in Washington State, specializing in service to Latino populations. The organization operates a network of more than 90 medical, dental, and behavioral health clinics and a wide variety of nutritional, social, and educational services. Sea Mar is a mandatory COVID-19 and flu vaccine organization. The Affordable Housing program recently acquired a multifamily apartment building in the Beacon Hill neighborhood of Seattle; the Supportive Services Specialist will support this building.

Position Summary

The Supportive Services Specialist will conduct initial screenings, provide comprehensive assessments, develop care plans, check in with residents, and consult with an interdisciplinary supportive services team. The productivity expectation is 15 or more client encounters per week for full‑time work with a caseload in excess of 65 low‑income, marginalized or otherwise disadvantaged clients, many of whom may have experienced homelessness.

Duties and Responsibilities
  • Exemplify excellent customer relations with clients, visitors, and employees; demonstrate courtesy, friendliness, and respect.
  • Identify problem behaviors and recommend interventions to reduce or eliminate those behaviors.
  • Provide brief, culturally relevant discussions covering crisis intervention, mental health concerns, pregnancy and birth options, grief, stress, parenting, coping skills, problem‑solving, abuse/neglect, and family violence.
  • Educate clients to make informed decisions regarding personal, child, and family health/mental health care and treatment.
  • Complete a thorough, culturally sensitive psychosocial assessment that includes personal history, family functioning, support systems, housing stability, and basic physical needs.
  • Ensure treatment planning and interventions are client‑centered, culturally competent, and age appropriate.
  • Make referrals to appropriate agencies or provide self‑referral assistance; assist clients in completing necessary forms and obtaining documentation.
  • Facilitate ongoing discussion of psychological, social, vocational, systemic, and economic goals and needs, then document each encounter and update assessments.
  • Chart notes of services provided as required by state and agency regulations.
  • Record and submit billable hours for encounters when applicable.
  • Monitor, compile, submit, and report program paperwork and data.
  • Participate in outreach services to prospective clients.
  • Work collaboratively with co‑workers and personnel from other agencies.
  • Maintain professional standards, follow treatment policies and procedures, and report unusual incidents immediately.
  • Perform other duties as assigned.
Qualifications
  • Good character and the emotional, physical, and social stability required to serve the population.
  • Knowledge of growth and development across the lifespan and the ability to identify age‑specific needs.
  • Ability to work effectively with an interdisciplinary care team (medical providers, nursing staff, care coordinators, behavioral health, preventive health, and other support staff).
  • Knowledge of evidence‑based screening tools (PHQ‑9, GAD‑7, AUDIT, DAST, etc.) and ability to use them for appropriate interventions and referrals.
  • Ability to establish and maintain professional rapport with patients and multidisciplinary team members.
  • Good working knowledge of electronic health record platforms and the Microsoft Office suite; ability to learn new programs as needed.
  • Typing proficiency of at least 35 words per minute.
  • No history of alcohol or drug misuse in the past three years; no felony convictions within the past seven years or any conviction of child abuse or physical harm.
  • Must pass a background check from the Washington State Patrol.
  • Must possess a valid driver’s license, current auto registration, and liability auto insurance coverage required by Washington state.
  • Must be fully vaccinated against COVID‑19 or have a valid medical or religious exemption.
  • Must pass a TB test at hire and annually; obtain Basic Life Support (BLS) CPR within 90 days of hire and maintain current certification.
Education, Certifications, and Licenses
  • Bachelor’s level degree in Counseling, Social Work, Psychology, or a related field (substitution of work experience will be considered on a case‑by‑case basis).
  • Preferred: Agency‑Affiliated Counselor in Washington state.
  • Preferred: Certified Peer Support Certification in Washington state.
