Job summary

DaVita seeks a registered nurse with palliative or hospice experience to assess patient needs, coordinate comprehensive care, and support medical plans across psychosocial and discharge planning. You will guide families, educate providers about IKC services, and help transition patients when appropriate, within a collaborative team.

Minimum two years in palliative/hospice care; RN license in practice states; experience in case management or healthcare coaching is preferred.

Qualifications

• 2-4 years experience.
• Associate's degree or apprenticeship.
• A.N.D. degree or higher from accredited school of nursing or three year diploma from accredited diploma programRequired
• Minimum of two years working in palliative or hospice care settingRequired
• At least one year of experience in healthcare performance coaching or case management Preferred
• At least two years of renal nursing experience Preferred
• RN license in state(s) of practiceRequired
• Certified Nephrology Nurse and/or Certified Case ManagerPreferred

Responsibilities

• Identifies and assists with the needs of patients including, but not limited to, medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members 20%
• Implements innovative nursing interventions appropriate for short and long-term needs of patients15%
• Ensures care coordination to complement and support medical treatment plans while integrated psychosocial, physiological, and discharge planning needs and post-discharge follow-up to prevent hospital readmission 15%
• Coordinates healthcare services to members across continuum of care; provides follow-up for member care and evaluates outcomes 15%
• Assists patients with identifying their goals, values, preferences and develops care plan that aligns with patient wishes10%
• Provides ongoing awareness and education of DaVita IKC services to providers and other teammates5%
• Assists patients potentially ready to transition from active to passive treatment/intervention, including educating patients on treatment options (i.e., hospice or palliative care) and connecting them with these resources10%
• Collaborates with and directs the day to day responsibilities of non-clinical teammates, as required (i.e. Integrated Care Assistant & Care Coordinator)5%
• Other duties as assigned. 5%
