Responsibilities

  • Oversee day-to-day nursing services for an assigned region or systems.
  • Ensure safe nursing services and medication administration across caseload.
  • Monitor and address program- and person-specific needs through nursing staff oversight.
  • Provide leadership and clinical support to program staff; primary nursing resource.
  • Develop knowledge of regulations, standards, and care requirements.
  • Direct supervise RN, LPN, and Regional Health Care Specialists.
  • Monitor staff performance and manage corrective/development plans.
  • Educate people supported on medical treatments and medications.
  • Travel to programs to assess care, collect data, and attend meetings.
  • Participate in hiring, recruitment, and retention initiatives.
  • Coordinate clinical training and documentation plans.
  • Monitor eMAR and EMR for documentation quality and compliance.
  • Address audit findings and implement corrective actions.
  • Lead nursing call list and triage calls during/after hours.
  • Lead intake assessments and placement recommendations.
  • Maintain compliance with HIPAA and other requirements.

Job description

  • Oversee and direct day-to-day nursing-related services for an assigned region and/or systems
  • Ensure safe and appropriate nursing services and medication administration across the assigned caseload
  • Monitor, assess, and address program- and person-specific needs through oversight of nursing staff
  • Provide leadership and clinical support to program staff and serve as the primary resource for nursing and medical needs
  • Develop and maintain knowledge of applicable regulations, nursing standards, and medical care requirements
  • Directly supervise Registered Nurses, Licensed Practical Nurses, and Regional Health Care Specialists
  • Monitor staff performance, conduct observations, and manage corrective or development plans
  • Ensure staff use the full nursing process and appropriately educate people supported about medical treatments and medications
  • Monitor nursing caseloads and recommend reassignments or staffing changes
  • Implement, monitor, and update nursing care plans and PONS; handle complex medical issues
  • Travel regularly to programs to conduct assessments, observe care, facilitate in-services, collect data, and attend meetings
  • Participate in hiring, interviews, staffing, recruitment, and retention initiatives
  • Process staff status changes and ensure accurate Workday timekeeping and approvals
  • Coordinate clinical training, assessments, documentation, and related plans
  • Plan and facilitate leadership and clinical team meetings
  • Monitor eMAR, electronic health records, and other medical systems for documentation quality and compliance
  • Analyze medical data and trends, including medication errors, 911 calls, and falls
  • Address audit findings and develop corrective action plans for continuous improvement
  • Participate in the nursing call list and triage calls during and outside regular working hours
  • Present information and respond to inquiries about the health and status of people supported
  • Lead intake assessments and support placement recommendations
  • Participate in a rotational residential nursing on-call system, including 24/7 response within 30 minutes when required
  • Maintain compliance with federal, state, local, OPWDD, HIPAA, infection-control, and other applicable requirements
  • Participate in agency committees, initiatives, medical in-service development, and special projects
Requirements
  • Valid, active Registered Nursing (RN) license and/or registration to practice as an RN in New York or New Jersey
  • Current Basic Life Support (BLS) certification for Healthcare Providers
  • Bachelor of Science in Nursing (BSN) degree and three (3) years of nursing experience in any healthcare setting, preferably with individuals with intellectual and developmental disabilities (I/DD); or satisfactory equivalent combination of education, experience and/or training
  • All incumbents must hold a current, valid RN license in the state of assignment and current BLS certification
  • Sound clinical judgment in selecting methods and techniques to resolve medical concerns
  • Excellent interpersonal, verbal, and written communication skills
  • Supervisory and leadership skills across multiple sites
  • Ability to lead, direct, and teach program staff on medical support, medication administration, and specialty care
  • Proficiency with computers, electronic medical records, billing and related electronic systems, email, Microsoft Office Suite, and ability to learn Workday
  • Ability to manage multiple priorities and deadlines
  • Must obtain and maintain additional certifications required for assigned programs, which may include SCIP-R, Diabetes Care, or Train-the-Trainer
  • Ability to travel between assigned programs, other regions, or local headquarters
  • Ability to work a flexible schedule, respond to or triage calls outside regular hours, and participate in on-call coverage
  • Ability to meet essential physical demands, including lifting/moving individuals and lifting up to 25 pounds on occasion
  • Preferred: one year supervising nurses in a formal or head nurse capacity
  • Preferred: two years supporting people with I/DD
  • Preferred: knowledge of I/DD, related diagnoses, and applicable state regulations
