Job summary

Harboroaks is seeking a dedicated supervisor to lead a patient support team in a care facility in Indianapolis. You will oversee daily operations, ensure compassionate patient care, and maintain safety and regulatory compliance.

The role requires a High School diploma (associate/bachelor in human services preferred) and 2+ years of experience in patient support, including 1+ year in a lead or supervisory position. CPR/de-escalation training is expected, with potential driving responsibilities.

Qualifications

Responsibilities

• Provide medical or therapeutic care per physician instructions.
• Coordinate the work of assigned staff and set day-to-day performance expectations.
• Guide and direct staff to foster trust, respect and teamwork.
• Assist patients with daily living activities and basic health care.
• Participate in therapeutic or recreational groups.
• Oversee admitting and discharging of patients.
• Monitor patients’ well-being and report unusual issues; maintain progress records.
• Observe and influence patient behavior through interaction, teaching and counseling.
• Encourage participation in social or therapeutic activities to build skills.
• Lead or co-lead individual or group sessions.
• Collaborate with staff to rehabilitate and return patients to the community.
• May provide transportation for patients as needed.

Overview

First shift: 7:00 am - 3:30 pm available

PURPOSE STATEMENT:

Direct, supervise and coordinate the activities of a patient support team.

Responsibilities

ESSENTIAL FUNCTIONS:

• Provide medical or therapeutic care, direct and indirect diagnostic and therapeutic procedures to patients, following the instructions of physicians or other health practitioners.
• Coordinate the work and activities of assigned staff, including setting performance expectations in the day-to-day operations of the unit.
• Provide guidance and direction to staff and encourage/build mutual trust, respect and cooperation among team members.
• Assist patients with their daily living activities and administer basic health care or treatments.
• Participate in therapeutic or recreational group.
• Oversee admitting and discharging.
• Monitor physical and emotional well-being of patients and report unusual behavior or physical ailments to appropriate staff. Prepare and maintain records of progress and services performed.
• Observe and influence the behavior of the patients by communicating, interacting with them, teaching and counseling.
• Encourage patients to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills or develop social relationships and teach strategies to promote patient wellness and independence.
• Lead or co-lead individual or group sessions as part of specific recreational or therapeutic procedures.
• Collaborate with other staff in working with the population of the facility to treat, rehabilitate and return patients to the community.
• May provide transportation for patients as needed.
• Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.

Qualifications

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:

• High School diploma or equivalent required.
• Associate's or Bachelor's degree in human service field from an accredited college or university preferred.
• Two or more years’ experience with the population of the facility in a patient support role required.
• One or more years’ experience as a lead or previous supervisory experience required.

LICENSES/DESIGNATIONS/CERTIFICATIONS:

• CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
• First aid may be required based on state or facility.
• May require a valid state driver's in facilities where transporting patients is an essential function.

ADDITIONAL REGULATORY REQUIREMENTS:

• Minimum 21 years of age. For Resource Treatment Facility a bachelor’s degree in a human service field; Associate degree with a minimum of 2 years of related experience; High School Diploma with a minimum of 4 years of related experience. Must have and maintain a valid Indiana driver's license and maintain automobile insurance coverage.
