Job summary

Aurora Health Care is seeking a field-based supervisor to drive quality patient care across home health operations. You will lead a team, ensure chart audits are completed, and act on identified trends to support operational goals.

The role emphasizes staff development, on-going education, and collaboration with managers to improve processes and maintain regulatory compliance. Travel is required.

Qualifications

  • Bachelor’s degree in Nursing or related field.
  • Valid driver's license with a registered, insurable car.
  • 5 years of clinical nursing or rehabilitation experience.
  • 1 year of supervisory experience preferred (staff/budgets).
  • Strong knowledge of home health or hospice regulations and reimbursement.
  • Excellent verbal and written communication skills.
  • Ability to organize, plan and delegate effectively.

Responsibilities

  • Promotes an environment conducive to quality patient care.
  • Assist with chart audits and ensure action is taken.
  • Identify trends in audits and report results to Manager and team.
  • Ensure field supervisory visits are completed according to policy.
  • Provide direction to staff for coordination of resources.
  • Monitor staff productivity against targets daily.
  • Correct charting deficiencies impacting revenue and operations.
  • Monitor clinical assessments, outcomes, and on-call utilization; advise leadership.
  • Participate in staff orientation, coaching, and on-going precepting.
  • Assess educational needs and facilitate in-services for staff.
  • Coordinate 24-hour availability of field staff and on-call rotation.
  • Conduct tracers and joint visits with ongoing feedback.
  • Collaborate with manager on process improvement.
  • Ensure timely OASIS data cycle time.
  • Work with operations to grow and maintain patient/referral relationships.
  • Maintain quality standards and evaluate workflows.
  • Develop relationships with customers, physicians, and Advocate Aurora Health at Home.

Major Responsibilities
  • Promotes an environment conducive to the delivery of quality patient care.
  • Assist with chart audits and assure action is taken.
  • Identifies trends in audits and communicates results to Manager and team.
  • Assures field supervisory visits are accurately completed according to agency policy as necessary.
  • Provides direction to the staff with clear responsibility for the coordination of resources to efficiently support day to day operations.
  • Monitors and assures staff productivity according to established targets on a daily basis.
  • Assures timely correction of charting deficiencies that impact revenue cycle and clinical operations by field team members.
  • Monitors and assures reliability of clinical assessments, tracks and remedies variances in outcomes and adverse events, monitors productivity and on-call utilization and makes recommendation to one up.
  • Participates in the orientation of staff through on-going direct teaching, coaching, observation, review and precepting of staff.
  • Assesses educational needs of clinical staff and assists in development and facilitation of in services for staff.
  • Coordinates and ensures 24 hour availability of field staff services and rotates in on call with manager.
  • Conduct tracers/joint visits and provide on-going feedback to staff.
  • In collaboration with the manager, conducts ongoing process improvement.
  • Ensure accurate, timely and thorough OASIS data cycle time.
  • Works collaboratively with operational staff and other leadership to ensure growth and maintenance of established patient and referral relationships.
  • Establishes and maintains quality standards, continually evaluating work flow and processes.
  • Develops collaborative and cooperative relationships with customers, physicians, community organizations and entities of Advocate Aurora Health at Home.
  • Assists in Field Team Interview/Hiring and Selection.
  • Licensure: Nurse, Registered (RN) OR Physical Therapist OR Occupational Therapist Education/Experience Required: Bachelor’s Degree in Nursing, or Bachelors Degree in Physical Therapy, or Bachelors Degree in Occupational Therapy, or Bachelors Degree in Speech Therapy.
  • Typically requires 5 years of experience in clinical nursing or rehabilitation.
  • Prefer 1 year of supervisory experience in management of staff and budgets.
  • Knowledge, Skills & Abilities Required: Must possess and provide proof of a valid Driver License issued by the state of Wisconsin or Driver License issued by the state of Illinois, and a registered, functional car with adequate insurance coverage.
  • Must have thorough understanding of home health or hospice regulations, reimbursement and care requirements.
  • Strong verbal and written communication skills.
  • Demonstrates an ability to organize, plan and delegate work effectively an efficiently.
  • Demonstrates an ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.
  • Demonstrates an ability to solve problems and make independent judgements and decisions based on experience and good judgement.
  • License issued by the state in which the team member practices.
Physical Requirements And Working Conditions
  • This position requires travel, therefore, will be exposed to weather and road conditions.
  • Operates all equipment necessary to perform the job.
  • Exposed to a normal office environment.
