Supervisor, Certified Clinically Advanced Nutrition Coordinator (CCANC) is an integral member of the care team supporting physician-led practice by driving clinical excellence, team-based care, evidence-based medical nutrition therapy, and clinical and emotional support for patients and caregivers. The CCANC builds cohesive relationships with physicians, nurses, social workers, pharmacists, case managers, and hospital staff to ensure seamless delivery of medical nutrition therapy, patient education, and clinical care. In collaboration with the medical team, the CCANC creates customized care plans, coordinates care transitions between hospital and home, and upholds department and hospital standards, objectives, and policies.
