Job summary

Wellpath seeks a Substance Use Disorder Counselor to evaluate, treat, and support individuals on their recovery journey within our facility. You will participate as an interdisciplinary team member to promote positive clinical outcomes and prepare clients for discharge with continuity of care.

Responsibilities include administering screening tools, delivering group and individual services, documenting meticulously per HIPAA, and contributing to data reports for contracts and accreditation.

Qualifications

• High School diploma and certification required.
• Bachelor’s degree in health services or related field preferred.
• Experience providing SUD counseling required.
• Independent practice encouraged.

Responsibilities

• Administer screening and assessment tools for SUD.
• Contribute to treatment plans and participate in MDT meetings.
• Deliver group and individual services for SUD patients.
• Document services thoroughly while protecting confidentiality under HIPAA.
• Plan continuity of care and discharge support.
