Job summary

C.O.R.E. Medical Clinic, Inc. seeks a dual-role Peer Support Specialist and Substance Use Navigator to assist patients in recovery, connect them to evidence-based treatments, and navigate services in Yolo/Sacramento contexts. Travel to jails, hospitals, and community sites is required with mileage reimbursement.

You will document according to state and federal rules, collaborate with a multidisciplinary team, and share lived experiences to motivate patients and families toward recovery goals.

Qualifications

• California Peer Specialist certification is required; training/certification may be completed upon written notice from the appointing authority.
• Valid CA driver license (Class C or higher), full vehicle registration and insurance at all times.
• Lived experience with substance use recovery strongly preferred and valued.
• Ability to communicate clearly and concisely, both verbally and in writing.
• Experience in community health, social services, or a clinical environment is preferred.

Responsibilities

• Provides information, education, and advocacy with technology to access recovery services.
• Maintains documentation per Federal/State requirements for Peer Support roles.
• Maintains up-to-date resource and referral information and relationships with sources.
• Connects patients to appropriate referrals and resources.
• Collaborates with SUNs, social workers, nurses, discharge planners, and jail staff.
• Learns multiple Electronic Health Record systems to facilitate referrals.
• Acts as a positive, stable role model and supports patients.
• Attends clinical/medical team meetings as needed for patient support/advocacy.
• Advocates for appointment services (child visits, court dates, medical/psychiatric, housing).
• Shares personal lived experiences in various settings to support recovery.
• Participates in committees/workgroups on behavioral health and related topics.
• Engages in community activities related to Behavioral Health/ Substance Abuse.
• Performs related duties; assists patients in understanding stigma.
• Follows C.O.R.E’s crisis policy; maintains confidentiality per CFR 42, HIPAA, Title 9.
