Job summary

Wellpath is seeking a Substance Use Disorder Counselor in Fresno to evaluate, treat, and support patients in recovery. You will work within an interdisciplinary team to ensure positive outcomes and discharge planning in compliance with Medi-Cal/CalAIM requirements.

The role emphasizes person-centered treatment, accurate documentation, crisis intervention, and collaboration with external providers for continuity of care.

Qualifications

• Experience providing SUD counseling required.
• Ability to practice independently preferred.
• Strong documentation skills and confidentiality.

Responsibilities

How you make a difference

The Substance Use Disorder Counselor evaluates, treats, and supports individuals in their recovery from substance use disorders. This role works as part of an interdisciplinary team to help support positive clinical outcomes and prepare individuals for discharge. The position promotes continuity of care by collaborating with care teams and supporting ongoing treatment needs. The counselor provides substance use disorder services to justice-involved individuals in alignment with Medi-Cal and CalAIM program requirements.

Key Responsibilities

• Conduct SUD screenings, assessments, and provide individual and group counseling while developing person-centered treatment plans.
• Focus on engagement, stabilization, relapse prevention, and readiness for continued treatment.
• Collaborate with interdisciplinary teams and contribute clinical input to discharge planning and continuity of care.
• Coordinate with external providers and agencies to support ongoing treatment and post-release services.
• Maintain compliant clinical documentation, provide crisis intervention, and ensure confidentiality requirements are met.

Additional Details

Ideal candidates will have hands-on experience providing substance use disorder counseling, including clinical assessments, individualized treatment planning, group and individual counseling, relapse prevention, crisis intervention, and coordination with multidisciplinary care teams. Experience working with justice-involved populations, Medi-Cal/CalAIM programs, MAT services, co-occurring behavioral health needs, and community treatment providers is highly preferred. Candidates should also be comfortable with detailed clinical documentation, treatment recommendations, warm handoffs, and supporting continuity of care following release.

Qualifications & Requirements

Education

• High School diploma and certification required.
• Bachelor's degree in health services field is preferred.

Experience

• Experience providing SUD counseling required.
• Ability to practice independently preferred.

Licenses/Certifications

• Certification or licensure in addiction counseling, such as CATC II or III, SUDCC II or III, CADC II or III, or a higher-level certification.
