Job summary

Qualifications

• Minimum high school diploma or equivalent.

Responsibilities

• Conduct comprehensive substance use screening, care planning, and treatment activities.
• Develop individualized care plans with clients and treatment team.
• Provide evidence-based services (e.g., CBT, MI, DBT) to caseload.
• Document progress notes and maintain accurate treatment records.
• Coordinate with medical providers, social services, and community resources.
• Perform toxicology testing and monitor client vitals as needed.
• Make rounds in residential units to monitor withdrawal management clients.
• Respond to crises and emergencies within scope of practice.
