Responsibilities

• Conducts initial intake screening and assessment to determine treatment needs.
• Creates individual treatment plans based on assessment findings and reviews progress.
• Provides clients with individual and group counseling and case management.
• Maintains up-to-date client records and prepares periodic reports as required.
