Job summary

Proforma Health in Wisconsin is seeking a Substance Use Disorder Counselor to provide individual and group counseling, conduct assessments, screenings, referrals, and develop treatment plans.

Responsibilities include case management, coordinating community supports, maintaining clinical documentation, supporting clients' recovery, collaborating with multidisciplinary teams, and engaging in supervision and professional development. Qualifications

• Valid Wisconsin SAC-IT, SAC, or CSAC credential.
• Associate's degree in human services or related field (bachelor's preferred).
• Knowledge of addiction, recovery, counseling, and case management principles.
• Experience with justice-involved populations and a valid driver's license preferred.
