A community organization is seeking a Substance Abuse Care Coordinator to oversee care for clients undergoing Medically Assisted Treatment. Job description

Shelter House is a community of residents, staff and volunteers working together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. The Substance Abuse Care Coordinator is responsible for coordinating care to Medically Assisted Treatment (MAT) clients and other appropriate levels of care based on AOD health assessment by developing, monitoring and evaluating interdisciplinary medical and behavioral health care plan.

Responsibilities
  • Orient and educate patients about the MAT process.
  • Monitor the delivery of care with the interdisciplinary team.
  • Evaluate the outcomes of patient care.
  • Facilitate CD educational groups.
  • Follow best practices and complete other assignments as directed by the supervisor.
  • Meet with client’s after assessment is completed by recovery counselor and discuss appropriate referrals and services based on assessment.
  • Enroll clients in Medicaid and obtain proper ID documents.
  • May complete assessments as needed.
Qualifications & Requirements
  • Must have a CDCA.
  • Having one of the following is helpful: Certified Peer Recovery Supporter (CPRS), Licensed Social Worker (LSW).
  • Knowledge and experience with homelessness, mental health and/or substance abuse and community resources.
  • Must pass criminal background check and have a clean MVR record.
  • Valid driver’s license with a good driving record, daily access to a private insured vehicle during work hours and the ability to be approved by Shelter House insurer.
Skills & Abilities
  • Ability to function effectively in a high energy environment.
  • Ability to work independently and work flexible hours.
  • Excellent written and verbal communication skills.
  • Possess self-initiative and demonstrated attention to detail.
  • Computer literacy with MS Office Suite. Experience with VESTA is a plus.
Physical Requirements
  • Time spent walking: 50%
  • Time spent on telephone and computer: 10%
