Job summary

Mount Sinai Health System in Queens, NY is seeking an experienced Advanced Practice Provider to serve as Stroke Program Coordinator for our Advanced Primary Stroke Center. The role blends clinical leadership, in-patient and outpatient care, teaching, and program oversight.

You will work under the Stroke Medical Director and Neurology leadership, coordinate tele-stroke and transfers, ensure adherence to stroke guidelines, and drive data-driven improvements within a diverse community setting.

Qualifications

• Master’s degree and clinical experience.
• Proficiency with database and report writing software; MS Office.
• Ability to remain positive under high stress.
• Excellent written and verbal communication.
• Professional handling of confidential information.
• Collaborate with nursing, physicians and administration.
• Understanding of coding, documentation and regulatory requirements.
• New York State License, DEA and Medicaid number.

Responsibilities

• Direct in-patient consultation services and outpatient care, plus teaching and supervision.
• Assist with stroke code activations and assess delays; participate in PI projects.
• Advocate for patients and families by designing policies that promote rights and care.
• Coordinate tele-stroke program and transfers among facilities.
• Identify stroke patients in real-time and enter data into the designated stroke center database.
• Prepare reports and reconcile data.
• Monitor operational efficiency of the stroke program.
• Participate in stroke program marketing.
• Collaborate on policies and protocols to improve outcomes.
• Provide coordination for the Stroke Steering committee.
