Job summary

Abrazo West Campus is seeking a Stroke Coordinator to provide clinical leadership in acute stroke care, serving staff, management, physicians, and patients. You will drive quality outcomes and coordinate education within a comprehensive stroke program.

You will support program development, assist with patient education, data analysis, and Joint Commission certification efforts, while mentoring staff in evidence-based practice.

Qualifications

  • Current clinical experience in Neurology and/or Neurosurgery.
  • Knowledge of program development and adult learning theory.
  • Strong communication and presentation abilities.
  • Ability to work with minimal supervision, prioritize and organize work.
  • Effective interpersonal and leadership skills.
  • Familiarity with PC-based software for word processing, spreadsheets, database and statistics.
  • Understanding data collection techniques and reporting methods.

Responsibilities

  • Identify and follow patients with acute stroke during hospitalization to apply evidence-based practice for outcomes.
  • Collaborate with Quality/Clinical Outcomes and Stroke Team to analyze data and drive improvement.
  • Communicate Stroke Program information to key stakeholders.
  • Coordinate preparation, certification, and recertification by Joint Commission for the Stroke Center.
  • Mentor staff in using evidence-based tools to improve stroke care.
  • Develop guidelines and protocols to advance care practices.

Job Summary

The Stroke Coordinator has clinical responsibilities and is a resource to staff, management, physicians, patients and other health team members regarding nursing care practice, with an emphasis in the area of acute stroke care. The major functions of the Stroke Coordinator are: (1) Professional consultation for Acute Strokes; (2) Drive Quality Clinical Outcomes; (3) Serve as a resource for patient education/staff development; and (4) Provide Program/administrative support.

  • Identify and follow patients with acute stroke during hospitalization for the purpose of applying evidence-based practice to improve patient outcomes, both physiological and financial. Participate in patient education and patient satisfaction survey completion at discharge
  • Collaborate with the Quality/Clinical Outcomes Department and members of the Interdisciplinary Stroke Team to analyze stroke-related data and establish performance improvement plan to achieve Perfect Stroke Care.
  • Effectively communicate relevant Stroke Program information to key stakeholders.
  • Practice and adhere to the "Code of Conduct" philosophy and "Mission and Value Statement".
  • Maintain hospital-wide communication regarding the Stroke Program.
  • Coordinate the preparation, certification, and recertification by the Joint Commission of the Stroke Center.
  • Identify and provide direction of system-specific problems requiring evaluation and change, and formulates recommendations regarding appropriateness and cost-effectiveness of solutions.
  • Mentor staff in using/fostering the use of evidence-based tools to improve the care of a stroke patient (stroke order sets, patient education tools.
  • Spearhead the continued development of improved care practices for patients, including practice guidelines and protocols through the use of evidence-based practice.

    • Ability to present clinical information to a wide variety of audiences;
    • Knowledge regarding program development, implementation, and evaluation;
    • Excellent interpersonal and communication skills, both oral and written are necessary;
    • Familiarity with PC-based software for word processing, spreadsheet, database and statistics is required; Effectiveness as a change agent and the ability to influence without direct accountability;
    • In-depth understanding of proper data collection techniques and reporting methods for analyzing and utilizing data;
    • Understanding good clinical guidelines principles.
  • Minimum Requirements
    • Education: Bachelor of Science in Nursing.
    • Experience:
      • Current clinical experience in Neurology and/or Neurosurgery;
      • Demonstrated current knowledge of program development, adult learning theory;
      • Strong communication skills and presentation abilities;
      • Prior experience demonstrating the ability to work with minimal supervision, prioritize and organize work, analyze data;
      • Must possess effective interpersonal and leadership skills
    • Licensure/Certifications: Arizona State RN License; BCLS & ALCS Training;
  • Preferred Requirements
    • Education: Masters Degree
    • Experience: Advanced Computer experience preferred
    • Licensure/Certifications: NIHSS Certification; CNRN, SCRN or NVRN-BC or attaining certification
