HonorHealth Medical Group is seeking a full-time Nurse Practitioner or Physician Assistant to join its outpatient stroke program at the HonorHealth Neuroscience Institute at Osborn in Scottsdale, AZ. The APP will work with two stroke neurologists, focusing on follow-up care for patients after stroke and other cerebrovascular conditions.

Responsibilities include evaluating patients post-stroke, coordinating post-discharge care, and developing secondary prevention plans.

Qualificações

• Certified Nurse Practitioner or Physician Assistant with current Arizona license.
• Neurology, stroke or related experience preferred.
• Strong clinical judgment and collaborative care approach.

Responsabilidades
