Job summary

Rafi Health is a field-based clinic serving Los Angeles' unhoused communities. We seek a full-time Physician Assistant (PA) or Nurse Practitioner (NP) with street medicine experience to deliver comprehensive care across encampments and shelters, providing trauma-informed primary, acute, preventive, and chronic care.

Qualifications

• Active, unrestricted California PA or NP license.
• Active DEA registration (or eligibility to obtain prior to start).
• Prior street medicine, mobile clinic, or field-based experience with unhoused or high-need populations.
• Strong clinical judgement and ability to work independently and collaboratively in field environments.
• Valid California driver's license and reliable transportation.

Job description

Rafi Health is seeking a full-time Physician Assistant (PA) or Nurse Practitioner (NP) with prior street medicine experience to deliver care directly to individuals experiencing homelessness across Los Angeles. This is a field-based role: you'll meet patients where they live — in encampments, shelters, and other outdoor settings — bringing high-quality, compassionate, trauma-informed care into the community.

Working collaboratively, you'll provide primary, acute, preventive, and chronic care while helping patients navigate their ongoing medical and social needs. This role is ideal for a clinician who is passionate about health equity, thrives outside the traditional clinic, and is committed to serving some of LA's most underserved communities.

Preferred Qualifications

• Experience with substance use disorder treatment, including buprenorphine/MOUD.
• Familiarity with LA-area homeless services, community resources, and referral networks.
• Bilingual (Spanish/English) or other second language.

Work Environment

This is a field-based, outdoor role. You'll deliver care in dynamic and sometimes unpredictable settings, be assigned to locations throughout Los Angeles, and adapt to varying weather and field conditions. Comfort working outside a traditional clinical setting is essential.

• Salary range: 150,000–180,000, commensurate with experience
