Job summary

The Masonic Homes of California is seeking a Director of Nursing Services (DON) for its Union City skilled nursing facility. You will lead nursing operations and ensure delivery of high-quality, resident-centered care, while maintaining regulatory compliance and driving quality improvement.

The role reports to the Administrator and partners with interdisciplinary leadership. Ideal candidates have an active CA RN license, 5+ years in SNF/long-term care, 3+ years in leadership, and familiarity

Qualifications

• Active California RN license required.
• Minimum 5 years of nursing experience in SNF or long-term care.
• At least 3 years in a leadership role (DON/ADON).
• Strong knowledge of Title 22 and CMS regulations.

Responsibilities

• Lead all nursing operations (RN, LVN, CNA).
• Ensure consistent delivery of high-quality, resident-centered care.
• Oversee assessments, care planning, and clinical documentation.
• Maintain strong clinical presence through rounding and team engagement.
• Lead survey readiness and response efforts.
• Recruit, hire, and develop nursing staff.
• Drive staff engagement, retention, and accountability.
• Oversee staffing, scheduling, and performance management.
• Lead QAPI initiatives and monitor key clinical indicators.
• Collaborate with physicians, consultants, and external partners.
