Job summary

Hospice of the Western Reserve's Reserve Care unit in Ohio seeks a compassionate Certified Nursing Assistant to provide direct patient care in home settings. You will assist with ADLs, monitor patient status, document care, and participate in team care conferences.

Under nurse supervision, you will support medication administration, care planning, and safety protocols while maintaining a patient-centered approach and adherence to agency policies and infection control measures.

Qualifications

• Proficiency in reading and writing English.
• State-tested nursing assistant certification in good standing.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Record activities provided and observations of patient at the time of visit in clinical records and appropriate documents.
• Attend and actively contribute during patient care conferences.

Job description

Reserve Care provides comprehensive hospice, palliative, and grief support to patients and families.

What You Will Do

• Provide personal care and assist with ADLs according to the patient’s plan of care.
• Provide a safe environment for patients.
• Safely ambulate/transfer patients as appropriate.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under the supervision of the nurse, assist patient with pre‑poured medications that are ordinarily self‑administered and prescribed by a physician under the supervision of a nurse (Home Care only).
• Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
• Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
• Record activities provided and observations of patient at the time of visit in clinical records and appropriate documents.
• Complete all charting, timesheets, and mileage logs as defined by agency policy and procedures.
• Attend and actively contribute during patient care conferences.
• Participate in HNA discipline meetings, team meetings, staff meetings, retreats, and in‑services as appropriate.
• Attend in‑services to obtain 3 education hours per quarter.
• Participate in the on‑call rotation as scheduled (Home Care only).
• Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.).
• Demonstrate positive customer‑service responsiveness according to agency standards.
• Be flexible in meeting patient/family needs that require variable shifts and weekends.
• Participate in chart review quality improvement activities and other meetings as assigned.
• Participate in coordination of and communication with volunteer staff.
• Provide basic caregiving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens.
• Maintain Transmission‑Based Precautions.
• Complete annual competency requirements.

Success Capabilities

• Proficiency in reading and writing English.
• State‑tested nursing assistant certification in good standing.
• Equivalent education/experience may substitute for minimum qualifications, except where legal requirements exist (e.g., license).
