Job summary

Northshore Home Health Care seeks an RN to join a dedicated nursing team in our stepdown unit. You will conduct comprehensive patient assessments, develop individualized care plans, and collaborate with interprofessional teams to ensure high-quality care.

Responsibilities include promoting patient safety, evaluating progress, and applying evidence-based practices while communicating effectively with patients and colleagues in a fast-paced clinical setting.

Qualifications

• Graduate of an accredited nursing program or equivalent for licensing.
• Current RN license in the applicable state, with relevant certifications such as BLS and ACLS or equivalent.
• No prior experience required.

Responsibilities

• Conduct comprehensive patient assessments gathering data from patients, families, and healthcare providers.
• Develop and implement individualized care plans, analyze patient data, and collaborate with interprofessional teams.
• Promote patient safety, evaluate progress, and ensure evidence-based practices are applied.
• Communicate effectively with patients and teams, maintain current knowledge, contribute to quality improvement.
• Perform other duties as assigned in a fast-paced clinical environment.
