The role emphasizes direct patient care, family education and collaboration with the interprofessional team to implement individualized care plans and ensure safety and regulatory compliance.

Qualifications

• Role involves direct patient care activities including assessment, planning, implementation and evaluation.
• RN licensure required.
• Education of patients and families on illness management and home care needs.
• Care plans developed and documented with interprofessional team.

Responsibilities
