Job summary

Froedtert Health is seeking a Staff RN for Ambulatory Clinics in West Bend, WI. This on-site role is 0.6 FTE with 24 standard hours per week, working Tue, Wed, Thu 08:00–16:30. You will join an interprofessional team to deliver patient- and family-centered care and coordinate care across settings.

The ideal candidate holds a Wisconsin RN license (or multi-state license) and a Bachelor’s degree is preferred. BLS certification is required within 90 days of hire.

Qualifications

• Current Wisconsin RN license or multi-state license.
• Bachelor's degree in Nursing preferred.
• AHA Basic Life Support (BLS) certification required or equivalent within 90 days of hire.
• Experience in ambulatory or clinical nursing is preferred.

Responsibilities

• Provide patient and family centered care as part of an interprofessional team.
• Coordinate care across settings with safety, effectiveness, and fiscal accountability in mind.
• Participate in quality improvement initiatives and professional development.
• Deliver direct patient care, education, and telephone triage/eVisits.

Job Summary:

The Staff RN-Ambulatory Clinics works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The Staff RN-Ambulatory Clinics systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The Staff RN-Ambulatory Clinics utilizes evidence to drive clinical decisions and practice related to individual and population health. The Staff RN-Ambulatory Clinics establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The Staff RN-Ambulatory Clinics considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The Staff RN-Ambulatory Clinics delegates and supervises care in accordance with the WI Nurse Practice. The Staff RN-Ambulatory Clinics participates in continuous quality improvement initiatives and assumes responsibility for their professional development. Major accountabilities of this role include patient care coordination, direct patient care, patient education, telephone triage/eVisits, and performing/assisting with in-clinic minor procedures. May float to other areas on as needed for operations need. Other duties as assigned.

EXPERIENCE REQUIRED: Clinical experience and/or interest in the care and management of patient populations specific to the area of employment.
EXPERIENCE PREFERRED: At least 1 year of relevant nursing experience
EDUCATION REQUIRED: Nursing licensure required.
