Job summary

Elara Caring is seeking a full-time Registered Nurse to provide home health care across Rhode Island, including Warwick and surrounding communities.

You will manage an assigned caseload, perform initial nursing evaluations, develop and update individualized plans of care, and coordinate with physicians, therapists, and caregivers to ensure safe, effective home-based care. Travel up to 50% is required.

Qualifications

• Current Rhode Island RN license.
• Bachelor’s degree in nursing.
• At least one year of nursing experience in a clinical care setting.
• Strong assessment, communication, organization, and patient-care skills.
• Ability to manage an assigned caseload and work independently in the field.
• Previous home health or hospice experience preferred.
• Willingness to travel up to 50%.

Responsibilities

• Manage an assigned caseload of home health patients.
• Complete initial nursing evaluations and ongoing reassessments.
• Develop, implement, and update individualized plans of care.
• Assess patients’ physical, psychological, social, environmental, and caregiver needs.
• Provide skilled nursing care in patients’ homes.
• Educate patients and caregivers about medications, treatments, disease management, and safety.
• Monitor patient progress and adjust care based on changing needs.
• Communicate changes in condition to physicians, clinical leadership, and other members of the care team.
• Coordinate services with nurses, therapists, physicians, caregivers, and community resources.
• Complete accurate and timely clinical documentation.
• Protect patient information and maintain compliance with HIPAA requirements.

• Previous Homecare Homebase experience is preferred
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.
• Must be able and willing to travel 50%
