Qualifications

• Active and unrestricted medical license.
• Unrestricted DEA license and dispensing license for state of jurisdiction.
• Must be eligible to participate in Medicare.
• FMCSA NRCME certification active or willing to certify during credentialing.
• Board Certification preferred.
