Job summary

Hennepin Healthcare's Home Health team in Minneapolis is hiring an RN for home visits to adults with acute and chronic conditions. This 1.0 FTE role involves day shifts, 80 hours per pay period, and every 4th weekend rotation in a setting that delivers care at patients’ homes.

Responsibilities include triaging symptoms, developing an individualized plan of care, educating patients and families, and ensuring continuity of care. Minnesota RN licensure and valid driving credentials are required.

Qualifications

  • At least 2 years of professional nursing (RN) home visiting/public health work experience.
  • Must have a valid driver's license and the availability of an insured vehicle to travel to patient's homes or agreed upon meeting places.
  • Graduation from an accredited school of nursing.
  • Possession of a valid license as a Registered Nurse issued by the State of Minnesota.

  • Protecting, promoting wellness and optimizing health
  • Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
  • Alleviating suffering through the treatment of the human response
  • Partnering with the patient, their family and significant others

  • Assuring continuity of care
  • Educating patient and family in preventative care which promotes health and well being

Home Health provides community health cae to patients wherever they call home. Services include home health visits for adults with acute and chronic medical conditions.

QUALIFICATIONS:
Minimum Qualifications:

  • EPIC proficiency and OASIS experience
