U.S. Renal Care is seeking a Clinical Nurse II in Santa Rosa to coordinate patient care as part of a SHC team. You will collaborate with care providers to ensure required care and act as a patient teacher and advocate, promoting informed consent and patient independence.

Qualifications
- Minimum of one (1) year of registered nurse experience.
- Graduation from an accredited nursing school or equivalent.
- Current state RN licensure.
- Current Healthcare Provider certification (CPR) required.
- Competent knowledge of Electronic Health Records.
- Ability to organize, prioritize and complete projects independently.

The Clinical Nurse II will discover and learn how to function as a SHC coordinator of patient care, collaborating with other care providers and health team members to provide the required care. The Clinical Nurse II acts as a patient teacher and advocate, assisting the patient in seeking information, assuring the patient has the opportunity for informed consent for treatment decisions and promoting the maximum level of patient-desired independence.

Experience:
