Milestone is seeking a Staff Nurse LVN for a part-time NOC shift in Mesa, AZ. You will provide high-quality clinical care, assess residents, update care plans, and communicate with families and staff while ensuring regulatory compliance.

The ideal candidate holds an Associate or Bachelor’s in Nursing, a valid nursing license, and at least 1 year of nursing or caregiving experience in assisted living or memory care. Strong communication and teamwork are essential for success in this role.

Qualifications

• Associate or Bachelor’s Degree in Nursing preferred.
• Valid State Nursing License (RN or LPN/LVN); required certifications as mandated by state.
• Minimum 1 year of nursing or caregiving experience, preferably in Assisted Living or Memory Care.
• Experience with Microsoft Word and Excel; familiarity with human resource rules a plus.
• Strong clinical assessment skills, conflict resolution, and problem-solving abilities.
• Excellent written, verbal, and interpersonal communication skills.

Responsibilities

• Assess residents’ health upon admission and ongoing changes, and develop or update individualized care plans.
• Communicate resident needs and changes to the Director, Health and Wellness, staff, and families.
• Participate in hands-on care and perform nursing tasks per residents’ care plans.
• Train and support care staff to meet continuing education and competency requirements.
• Transcribe and review physician orders; monitor medication administration and MAR reviews.
• Ensure compliance with state, federal, and local regulations, including HIPAA, OSHA, and Bloodborne Pathogen Guidelines.
• Participate in CQI initiatives and report issues to leadership.
• Work safely and model safe practices for staff; correct unsafe actions as needed.
• Foster teamwork, show initiative, and positively impact coworkers, residents, and visitors.
• Report elder maltreatment or abuse in accordance with policy.
• Perform other duties as assigned.

About the Role

We're seeking a Staff Nurse LVN Part-Time NOC Shift to provide high-quality clinical care and support for residents in our Assisted Living and Memory Care communities. In this role, you'll assess resident health status, develop and update care plans, communicate with staff and families, and ensure compliance with all regulatory standards. You'll foster a safe, compassionate, and engaging environment while supporting the professional growth of the care team.
