Northwestern Medicine in Huntley, IL is seeking a Staff Nurse-Infectious Disease for a full-time day shift. The role covers both Huntley and McHenry clinics, 40 hours per week, Monday through Friday, 8:30am-5:00pm.

The nurse will provide comprehensive care to acutely ill patients, perform phone triage, educate patients and families, and coordinate discharge planning. The position adheres to ANA standards and emphasizes patient safety, quality of care, and collaboration with the health-care team

Qualifications

• Current nursing license in Illinois required.
• Graduate from an accredited school of nursing.
• AHA BLS certification required.
• BSN degree preferred if stated.

Responsibilities

• Provide nursing care to acutely ill patients using the nursing process.
• Administer medications following six rights and clinical guidelines.
• Perform phone triage and deliver test results with care instructions.
• Assess and update plans of care, educate patient and family, and involve them in the plan.
• Support discharge planning and participate in quality improvement activities.

Education

Registered Nurse licensure in Illinois

American Heart Association Basic Life Support CPR certification

Effectively interacts with patient, significant others, and other health team members, while maintaining ANA standards of professional nursing practice. Also responsible for phone triage, abnormal test result delivery and corresponding care instructions.Responsibilities:- Performs all aspects of patient care by utilizing evidenced based practice in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors.- Follows the six medication rights and reduces the potential for medication errors, administering medications based on the age and clinical condition of the patient.- States rationale for clinical decisions based on critical thinking.- Assesses, analyzes, validates and integrates data to assess patients' physical psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs.- Identifies patient care needs, problems and variances. Identifies patient care needs, problems and variances and incorporates into care plan.- Identifies the patient's learning needs, abilities, preferences and readiness; also, identifies support systems, coping mechanisms and barriers to learning.- Explains procedures, therapies and treatments to patient and family, evaluates and documents patient and family response to teaching.- Uses appropriate resources to provide education.- Involves patient/family in the formation of the plan of care.- Develops, implements and updates an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care and discharge planning.- Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques.
