Job summary

Adventist Health Portland seeks a Pharmacist to compound, prepare, mix, manufacture and dispense medicines with high technical and organizational skills. The role emphasizes safe, efficacious therapy and proper handling of drugs within the hospital pharmacy.

The Pharmacist will supervise and train technical personnel, participate in clinical rounds, and collaborate with leadership to optimize formulary, costs, and procurement while maintaining patient care standards.

Qualifications

  • Graduate of school of pharmacy is required.
  • Bachelor's Degree preferred.
  • Two years' technical experience preferred.
  • Two years' hospital pharmacy experience preferred.
  • Pharmacist license in the state of practice required.
  • All licenses and certifications needed to fulfill duties required at the state level.

Responsibilities

  • Processes, enters, verifies and sends medication orders in a timely manner per policies and regulatory guidelines.
  • Ensures medication therapy is efficacious, safe, and cost effective.
  • Establishes best therapeutic outcomes and optimal prescribing.
  • Conducts daily reports on medication utilization to identify improvement areas.
  • Participates in rounds, emergency department tasks, and other department programs.
  • Assists in cost savings and formulary compliance.
  • Collaborates with leadership on purchasing and contracting for medications and supplies.

Education

Graduate of school of pharmacy
Bachelor's Degree
Two years' technical experience
Two year's experience as a clinical pharmacist specializing in a regulatory area
One year's hospital pharmacy experience

Job Summary

Compounds, prepares, mixes, manufactures and dispenses medicines using a high level of technical and organizational skills. Uses critical thinking abilities. Oversees the proper handling, usage and storage of drugs in the pharmacy. Maintains standards set by the department and translates policies/procedures into daily operation and clinical activities. Supervises and trains assigned technical personnel and assumes responsibility for technical support activities within the pharmacy department.

  • Graduate of school of pharmacy: Required
  • Bachelor's Degree: Preferred
  • Two years' technical experience: Preferred

Essential Functions

Processes, enters, verifies and sends medication orders within the institution in a timely manner that is in compliance with department/hospital policies, Joint Commission and other regulatory guidelines. Ensures medication therapy is efficacious, safe, convenient and cost effective. Establishes the best therapeutic outcomes. Assures that medication prescribing is optimal with respect to indication, dose, route, frequency, history of allergy, and does not duplicate or interact with the effects of other drugs. Conducts and manages daily reports on medication utilization based upon daily rounds to identify, catalogue and subsequently analyze areas in which medication improvement is needed. Performs quality assurance procedures in related work areas. Participates in all pharmacy-related tasks and programs such as clinical rounds, emergency department, short-staffed areas, and all other areas as directed/necessitated. Expedites changes needed to meet regulatory, accrediting, licensing and government agency standards and laws. Participates in department cost savings. Complies with drug formulary and appropriate documentation. Collaborates with appropriate leadership and buyer to purchase, contract, and procure medications and related supplies, ensuring product and vendor selection is based upon the highest medication safety standards. Requirements
