Job summary

Responsibilities

• Provide comprehensive assessments, evaluations, and diagnostic summaries of chemically dependent patients.
• Compile patient-centered, comprehensive treatment plans, meeting regulatory time frames.
• Document patient progress towards the goals of treatment within OASAS regulatory standards.
• Conduct education and therapeutic treatment protocols focused on patient needs and OASAS regulations.
• Provide case management services, in behalf of patient with all collateral agencies. Provide necessary reports to TASC, Drug Court, Parole, DCMH, DSS, CPS.
• Complete discharge summaries in the OASAS time frames, with appropriate aftercare in place.
• Bi-lingual in Spanish required.
