Qualifications

• Bachelor's Degree in Physical Therapy.
• Florida Physical Therapy licensure - active and in good standing.
• American Heart Association BLS - active and in good standing.
• Level II fingerprinting background check initiated within 15 days and finalized within 90 days.

Responsibilities

• Participates in interdisciplinary/multidisciplinary meetings related to patient care.
• Maintains and documents productivity standards.
• Records daily patient documentation, statistics, and charges accurately and timely.
• Improve access to care for the patients served.
• Conducts evaluations and provides treatment recommendations based on findings.
• Communicates progress with patients, families, and care team.
• Develops a care plan with measurable treatment goals.
• Recommends and provides treatments based on findings.
• Prepares patient/family for discharge when therapy benefit is reached.
• Follows discharge protocols and documentation.
• Provides evidence-based practice CEUs and peer review as required.
• Maintains ongoing CEUs for licensure and clinical excellence.
