AdventHealth in Altamonte Springs, FL seeks a full-time Spiritual Care Chaplain to assess spiritual needs, manage referrals, and develop care plans for patients. Strong communication and multi-tasking abilities are required. The role involves documenting in Salesforce and collaborating with the E-Spiritual Care Center.

Position requires a Bachelor's in theology/healthcare, ACPE credentials, and CSC/CPE experience. Competitive benefits and day-shift schedule offered.

Qualifications

• Bachelor's degree in theological studies, healthcare or related field.
• ACPE credential required.
• Minimum one unit of Clinical Pastoral Education (CPE).
• Two or more CPE units preferred.
• Board Certified Chaplain (BCC) preferred.
• Ordained Minister (ORDM) preferred.
• Level 1A CPE completion required.
• Florida background screening as applicable.

Responsibilities

• Assesses spiritual needs of patients and provides appropriate interventions.
• Receives referrals and follows up within 48 hours.

• Develops spiritual care plans with objectives and outcomes.

Job Description:

• Demonstrates through behavior the organization's mission, vision, values, and service standards.
• Assesses spiritual needs of patients and provides appropriate interventions considering growth development needs and age group.
• Receives referrals from the E-Spiritual Care Center manager and follows up within 48 hours.
• Responds to incoming calls and conducts necessary follow-up on inquiries. Completes referral activity logs in the Salesforce Customer Relationship Management system.
• Develops spiritual care plans for patients by identifying objectives, interventions, and desired outcomes.
• Receives and distributes messages to outpatient entity staff and service providers.
• Communicates appropriate information for patient’s medical record through documentation on the Spiritual Care Consult form.
• Utilizes knowledge of different levels of growth, development, and skills for various ages and demographics of patient care.
• Available for various shifts as scheduled by the Spiritual Care Manager.
• Performs other duties as assigned.
