Job summary

Vivie is seeking a part-time Spiritual Care Counselor in Minnesota. The position offers 20 hours per pay period, Monday to Friday, 8 a.m. to 4:30 p.m., with a pay range of $22.00–$25.00 per hour based on experience.

You will provide spiritual and emotional support to patients and families, liaison with community clergy, participate in hospice care planning, assist with crisis and bereavement support, document patient plans, and travel to client homes within the region.

Qualifications

• Valid driver’s license and access to an insured personal vehicle.

• Ability to read, write, and speak English to ensure effective communication with staff, patients, and families.

Responsibilities

• Provide Spiritual & Emotional Support - Offer prayer, life review, and counseling to patients and families, adapting care to individual beliefs and preferences.
• Serve as a Resource & Liaison - Build relationships with community clergy and connect patients with spiritual care services as needed.
• Participate in Interdisciplinary Team Meetings - Collaborate with hospice staff to integrate spiritual care into patient-centered care plans.
• Offer Crisis Intervention & Bereavement Support - Assist families and caregivers through grief, trauma, and stress, providing compassionate guidance during difficult times.
• Document & Maintain Patient Care Plans - Ensure accurate and timely documentation of patient needs, interventions, and family support efforts
