Qualifications

Responsibilities

• Select, administer, and interpret clinically appropriate assessments.
• Integrate educational, medical, social, and psychological factors when evaluating patients and determining treatment diagnoses.
• Develop and implement individualized plans of care that promote skill development and independence.
• Evaluate treatment outcomes and modify plans of care as needed.
• Design home exercise programs and provide instruction to patients and caregivers.
• Maintain accurate documentation including treatment notes and referrals.
• Ensure compliance with HIPAA and professional ethics.
• Collaborate with healthcare teams to coordinate services and maximize outcomes.
