Assured Home Health, part of the LHC Group within the Optum family, is seeking a Speech Therapist to evaluate and treat patients with speech, language, swallowing, and cognitive disorders in home health settings.

You will develop care plans, coordinate with the health care team, and provide patient and family education while documenting progress and outcomes. This role requires Oregon licensure, a valid driver's license, and CPR certification or availability within 90 days of hire.

Qualifications

• Licensed in Speech Therapy in Oregon.
• Current driver’s license and access to reliable transportation.
• CPR certification preferred or within 90 days of hire.

Responsabilités

• Evaluates and treats patients to improve communication, swallowing, and cognitive function.
• Develops and implements a plan of care in collaboration with the health care team.
• Monitors progress and documents patient responses to therapy.
• Educates patients and families on therapy techniques and home practice.
