Job summary

Palms Care Center in Chowchilla, CA is seeking a Speech-Language Pathologist (CFY) to join our interdisciplinary team. We welcome experienced SLPs and Clinical Fellows who are passionate about helping residents regain communication, cognition, and swallowing function to improve quality of life.

You'll provide evidence-based, individualized treatment plans, collaborate closely with clinicians, and contribute to a supportive, growth-oriented environment with mentorship and supervision for CFYs.

Qualifications

  • Master's degree in Speech-Language Pathology required.
  • California license (or eligibility) required.
  • CFY applicants encouraged to apply.
  • CCC-SLP preferred but not required.
  • Strong communication and interpersonal skills.
  • Passion for working with older adults in a skilled nursing environment.
  • Commitment to providing compassionate, resident-centered care.

Responsibilities

  • Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.
  • Develop and implement individualized treatment plans.
  • Perform bedside and clinical swallowing evaluations.
  • Collaborate with physicians, nursing, OT, PT, dietary, social services, and families.
  • Maintain accurate documentation in line with Medicare, state, federal, and facility requirements.
  • Participate in interdisciplinary team meetings and care planning.
  • Educate residents, caregivers, and staff on communication and swallowing strategies.
  • Promote the highest level of resident independence and quality of life.

Position Summary

As a Speech-Language Pathologist, you'll help residents regain communication, cognition, swallowing function, and independence while improving their overall quality of life.

You'll provide individualized, evidence-based treatment plans and collaborate closely with our interdisciplinary team to deliver exceptional outcomes.

Primary Responsibilities
  • Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.

  • Develop and implement individualized treatment plans.

  • Perform bedside and clinical swallowing evaluations.

  • Collaborate with physicians, nursing, occupational therapy, physical therapy, dietary services, social services, and families.

  • Maintain accurate and timely documentation in accordance with Medicare, state, federal, and facility requirements.

  • Participate in interdisciplinary team meetings and care planning.

  • Educate residents, caregivers, and staff regarding communication and swallowing strategies.

  • Promote the highest level of resident independence and quality of life.

Qualifications
  • Master's Degree in Speech-Language Pathology

  • Current California Speech-Language Pathologist License (or eligibility)

  • Clinical Fellows (CFYs) are encouraged to apply

  • Certificate of Clinical Competence (CCC-SLP) preferred but not required

  • Strong communication and interpersonal skills

  • Passion for working with older adults in a skilled nursing environment

  • Commitment to providing compassionate, resident-centered care
