Job summary

The Center at Lincoln in Colorado is seeking a compassionate Speech Therapist to join our patient-centered team. You will provide speech and language therapy, evaluate needs, and collaborate with physicians and care staff to develop and execute therapy plans.

The ideal candidate holds a Colorado license in Speech Therapy, current CPR certification, at least 1 year of home health experience, and proficiency with EMR systems.

Qualifications

• Must be currently licensed in Speech Therapy in Colorado.
• Current CPR certification required.
• Minimum of 1 year home health experience.
• Must be able to use EMR systems (Kantime preferably).

Responsibilities

• Build in-person, trusted, therapist-patient relationships.
• Assess and evaluate patient care needs related to speech and language disorders and related training.
• Provide services within scope of practice as defined by state laws and plan of care.
• Evaluate function using diagnostic and prognostic tests.
• Assist physician and care team in developing the therapy plan of care.
• Treat patients to communicate effectively according to patient condition.
• Instruct patient, family and caregivers in speech therapy areas.

Speech Therapist Job Summary: Provide Speech Therapy in accordance with current federal, state, and local standards, guidelines, and regulations that govern our organization.

Speech Therapist - Essential Job Functions

• Build in-person, trusted, therapist-patient relationships
• Assessment and evaluation of patient care needs related to treating speech and language disorders and functional training in communication, swallowing, and cognitive impairments.
• Provide services within the scope of practice as defined by the state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Evaluate the patient's level of function by applying diagnostic and prognostic functional ability tests.
• Assist the physician and care team in the development of the therapy plan of care.
• Treat patients to communicate effectively by expressing thoughts according to the patient's condition using acceptable standards of practice.
• Instruct the patient, the family and/or caregiver and other members of the health care team in areas of speech therapy in which they can participate.
