Job summary

Regency Florence, a skilled nursing facility in Florence, Oregon, seeks a licensed Speech-Language Pathologist to join our in-house therapy team. You will evaluate and treat residents with speech, language, swallowing, and cognitive disorders, coordinating with physicians and care staff.

You will perform initial evaluations, document OASIS assessments, and develop treatment plans in collaboration with the care team.

Qualifications

• Graduate in Speech-Language Pathology with state licensure or eligibility.
• Experience evaluating and treating speech, language, swallowing disorders.
• Familiarity with OASIS assessments and interdisciplinary care.

Responsibilities

• Plans and administers medically prescribed speech therapy treatments.
• Evaluates speech and language skills considering medical and psychosocial factors.
• Reviews orders and records to determine required therapy services.
• Performs initial evaluations and documents OASIS assessments as needed.
• Plans written treatment plans with physician, patient, and caregiver input.
• Delivers rehabilitative plans to improve communicative function.

Services Offered

Skilled Nursing, Rehabilitation Therapy, Long-Term Care, and Respite Care

The Speech Therapist evaluates and treats residents and patients with temporary or long-term speech, hearing, swallowing and cognitive disabilities in compliance with state and federal guidelines.

You Will

• Plans and administers medically prescribed speech therapy treatment for patients with speech, language, and swallowing problems.
• Evaluates speech and language skills as related to educational, medical, social, and psychological factors.
• Reviews referral, physician’s orders, patient's condition, and medical records to determine speech therapy treatment indicated. Accepts physician orders regarding speech therapy services.
• Performs initial patient evaluation before initiating therapy. Performs and documents OASIS assessments at appropriate time points.
• Plans and prepares written treatment plans based on evaluation of patient data in conjunction with the physician, patient, and caregiver(s).
• Plan direct or conduct rehabilitative treatment plans to restore communicative efficiency of individuals with communication problems of organic and non-organic etiology.
• Other duties as requested.

You Currently

• Education and/or Experience: Graduate of an accredited school of speech pathology and language disorders.
• Licensed as a speech-language pathologist by the State Board of Speech-Language Pathology and Audiology or the Accrediting Body.
