Job summary

Sioux Center Health is seeking a Speech-Language Pathologist to evaluate patients, develop treatment plans, and provide therapy addressing speech, language, cognition, voice and dysphagia in multiple care settings.

The role requires CCC-SLP or active ASHA certification and a state license, with preference for hospital, SNF, outpatient, or home health experience; strong teamwork and documentation skills are essential. PRN coverage for weekends/holidays possible based on needs.

Qualifications

• CCC-SLP or ASHA-certified or in supervised fellowship with certification within one year of employment.
• Active state license to practice Speech-Language Pathology on hire.
• Hospital, SNF, outpatient or home health experience preferred.

Responsibilities

• Evaluate patients and interpret diagnostic assessments to guide intervention.
• Develop and implement individualized treatment plans.
• Provide direct therapy across settings addressing speech, language, cognition, voice and dysphagia.
• Coordinate with rehabilitation, medical and nursing care plans.
• Document evaluations, progress notes and treatment results.
• Communicate patient status with family, physicians and team members.
• Participate in department program development and intern supervision.

A Brief Overview

Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served.

What You Will Do

• Evaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine the levels of functioning and intervention needs in all areas of speech pathology.
• Demonstrates knowledge of age-specific factors and performs satisfactorily related to patient's age infant through geriatrics as noted on unit specific guidelines and competencies.
• Develops and implements individualized treatment programs for the patients needing services.
• Establishes and modifies intervention goals and strategies for patients/ treatment program based on on-going evaluations. Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals.
• Provides direct therapy services to patients using various functional settings and strategies.
• Plans therapy with in the patient's individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans.
• Prepares written documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports.
• Communicates effectively with patient, family, physician, and other disciplinary team members regarding the patient status, progress, and needs.
• Establishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient's intervention program.
• Participates in program development of the department. Evaluates and monitors clinical internship with students affiliating in various universities/colleges.

Required Education, License/Certification, Or Work Experience

• CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment Upon Hire or
• Speech-Language Pathologist - Licensing Board An active license in the state of practice Upon Hire

Preferred Education, License/Certification, Or Work Experience

• 1-3 years Speech Language Pathologist in a hospital, SNF, outpatient and/or home health setting
