DayToDay Health seeks a Speech Pathologist to evaluate, develop, and implement individualized treatment plans for patients with speech, language, cognition, voice, and dysphagia deficits in home settings. You will assess, plan, and deliver therapy across ages, from infants to geriatrics, coordinating with families and physicians to ensure comprehensive care.

Qualifications

  • ASHA CCC-SLP or active licensure in the practice state.
  • 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
  • Strong communication, teamwork and interpersonal skills.

  • Create and modify personalized therapy programs, setting achievable goals.
  • Provide direct therapy services tailored to patients’ physical, emotional, and cognitive abilities.
  • Coordinate with multidisciplinary teams, including families and physicians, to ensure comprehensive care.
  • Document evaluation results, treatment plans, progress notes, and other reports accurately.
  • Participate in program development and supervise clinical interns from educational institutions.
  • Communicate effectively with patients and team members to monitor progress and adjust interventions.

Key Responsibilities

  • Provide direct therapy services tailored to patients’ physical, emotional, and cognitive abilities.

Qualifications & Skills
  • Certification from ASHA (CCC-SLP) or active licensure in the practice state.
  • Preferred 1-3 years of experience in hospital, SNF, outpatient, or home health settings.
  • Strong communication, teamwork, and interpersonal skills.
  • Ability to perform essential functions with visual acuity and sound communication.
