Tender Care Home Health & Hospice in Las Cruces, NM seeks a Speech-Language Pathologist to implement standards of care and support patient communication goals within a home health setting.

Qualifications

  • Requires a Master’s in Speech-Language Pathology with internship.
  • ASHA CCC eligibility or certification and state licensure.
  • Current CPR certification.
  • Strong communication and documentation skills.
  • Experience in home health or clinical settings is preferred.

Responsibilities

  • Assist physician in evaluating patient function and homes environment.
  • Set goals and plans of care to improve speech/language function.
  • Complete recertifications, resumption-of-care, and discharge assessments per policy.
  • Educate patient and family on communication methods and device options.
  • Document evaluations, plan of care, and progress reports accurately.

Job Description Summary

The speech–language pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

Job Duties (include, but are not limited to the following):
  • Assists the physician in evaluating the patient’s level of function.
  • Assesses home environment and identifies equipment needs related to speech/language function.
  • Identifies functional speech deficits and establishes goals and plan of care to improve patient’s function, revising as necessary.
  • Performs and complete recertifications, resumption-of-care, and discharge Comprehensive OASIS assessments and associated paperwork in accordance to agency policy.
  • Instructs patient and family members, as well as other health team members, in methods of assisting the patient in improving communication skills.
  • Recommends communication devices/aids as indicated.
  • Periodically re-evaluates and upgrades the patient’s program to maximize functional improvement.
  • Records and reports to the patient’s physician the patient’s progress in treatment and any changes in the patient’s condition and plan of care.
  • Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
  • Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
  • All patient visit notes must be turned in the next scheduled work day, not to exceed forty-eight (48) hours.
  • Maintains necessary statistical and billing data.
  • Keeps abreast of changes in speech pathology practice and theory, contributes to the field through research, presentations, and in-services.
  • Keeps his/her immediate supervisor informed of problem areas and makes suggestions for improvement.
  • Attends and contributes to in-services, clinical records reviews, meetings required to ensure coordinated and comprehensive plans of care for the patients of the agency.
  • Complies with Homecare Agency Compliance Plan.
  • Complies with Homecare Agency’s Infection Control Policies.
  • Complies with Homecare Agency’s HIPAA Policies.
Position Qualifications:
  • Possesses a degree that includes an internship from an approved Master’s program in Speech-Language Pathology, or as required by applicable law and/or regulation.
  • Meets the education and experience requirements for a Certificate of clinical Competence in Speech-Language Pathology granted by the American Speech-Language-Hearing Association (ASHA).
  • Licensed in the State of practice by the State Board of Healing Arts to practice as a Speech Pathologist.
  • Is certified to practice Speech-Language Pathology.
  • A minimum of one year’s clinical experience in Speech-Language Pathology.
  • Demonstrates good verbal and written communication, and organizational skills.
  • Possesses and maintains current CPR certification.
