Qualifications

• Must hold a valid Speech Language Pathology license in the practice state or be eligible to obtain a degree from an accredited SLP program.
• Proficient in documenting in an EMR and compliance with regulatory standards.
• Ability to drive to patient homes and senior living communities as part of the Geriatric House Calls model.

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in homes or senior living settings.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Offer telehealth/virtual care when appropriate to extend access.

Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain Degree from an accredited Speech Language Pathology program Basic computer and EMR proficiency

• Providing moderate to maximum assistance with patient transfers and bed mobility
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model
